If you're requesting an Authorized Certified Copy of a California birth certificate, your application may require a notarized sworn statement verifying that you're legally authorized to receive the record.

Please note:
The notary does not notarize or certify your birth certificate.
Instead, when requesting an Authorized Certified Copy, an eligible requester completes a Sworn Statement declaring under penalty of perjury that they are legally authorized to receive the certified record. The required notarial certificate is then completed by the notary before the application is submitted.
The process of acquiring a certified copy of a birth certificate typically goes like this:
Complete Application → Sign Sworn Statement with Notary → Submit Request → Vital Records Issues Certified Copy
Authorized Certified Copy
An authorized copy can be used to establish the identity of the person named on the certificate and is available only to individuals authorized under California law. A notarized sworn statement is required for applicable mail requests.
Certified Informational Copy
An informational copy contains essentially the same record information but is marked “Informational, Not a Valid Document to Establish Identity.” It does not require the same notarized sworn statement.
Not sure which type you need? The agency or organization requesting your birth certificate can tell you which version is required. As a notary public, I cannot advise you which type of certified copy you should obtain.
Who Can Request an Authorized Certified Copy?
California limits Authorized Certified Copies to certain eligible requesters, which may include the person named on the certificate, certain immediate family members, legal representatives, attorneys representing the registrant or estate, and other persons authorized by law or court order.
If you're unsure whether you're eligible, check with the government agency issuing the record before your notary appointment.
California CDPH Authorized vs. Informational Copies
Requesting a California Birth Certificate by Mail
For a California state request, CDPH currently instructs applicants requesting an Authorized Certified Copy by mail to submit the birth-record application, the required notarized sworn statement, and the applicable payment. The current state fee is $31 per copy.
Sonoma & Marin County Birth Certificate Requests
Sonoma County
Sonoma County's current birth-record application states that an authorized requester submitting the application by mail or fax must have the signature on the Sworn Statement acknowledged by a Notary Public.

Marin County
Marin County similarly requires someone requesting an Authorized Certified Copy by mail or online to sign the Sworn Statement and have the notarial certificate completed by a Notary Public. In-person applicants can instead sign before county staff and present identification.

Can a California notary certify a copy of my birth certificate?
No. Certified copies of California birth records are issued by authorized government agencies. A notary may notarize the sworn statement associated with a request for an Authorized Certified Copy.
Do I need a notary to order a birth certificate?
Not always. The requirement depends on the type of copy and how you're requesting it. For example, CDPH requires a notarized sworn statement when requesting an Authorized Certified Copy by mail.
Can you come to my house to notarize my birth certificate request?
Yes. 415 Notary is a mobile service, so I can meet you at your home or another convenient location in my service area.
Can you get the birth certificate for me?
No. I provide the notarization required for the sworn statement. The birth certificate itself is requested from and issued by the appropriate state or county agency.
Can you notarize a request for a birth certificate from another California county?
Potentially, yes. The notarization can generally be performed locally even when the record is held elsewhere in California. You should follow the specific instructions provided by the agency holding the record.
Does an informational birth certificate require notarization?
CDPH says the notarized sworn statement is required for an Authorized Certified Copy, but not for a certified informational copy.
